Since Greg M had a hack at this and pointed out some trouble spots I decided to make a run at this image and fix the issues he brought up.
My work flow:
1) Open the image in CM using LAB and set highlight and shadow thresholds using the threshold feature of CM. Apply and close CM
2) Open the image in CM again in RGB mode and set three hue clocks on a shadow, Highlight and Mid-tone that should be gray. Used the BTN process to set the shadow and highlight as shown in Shot1 and calculated the average for the mid-tone.
3) Adjusted the mid-tone and tweaked the Shadow and Highlight to make all three hue clocks as shown in Shot2. Apply and close CM.
4) Open the image in CM in LAB mode again and adjust the overall contrast and lighting as shown. Shot4 . Also increased the saturation using the slider. Apply and close.
5) Created a layer copy and sharpened using unsharp. Settings were // 250% / .7 / 0 / set layer at 75%.
Total time 4 minutes...
